Payment and Participation
I agree to pay Shakespearean Youth Theatre (hereinafter referred to as SYT) the full amount as advertised for this class/workshop/camp/other program (hereinafter referred to as the Activity) at the time of this registration, unless otherwise discussed and agreed to in writing by SYT Staff or Officers. It is understood and agree that I will submit payment(s) pursuant to the specific payment deadlines as outlined in the email message sent to me at the time of registration.
Cancellation and Refunds
SYT will refund payments in full in the event that the Activity is cancelled by SYT, its Staff or Affiliates. In the event of a cancellation by the participant, SYT will refund one hundred percent (100%) of tuition payments when notified more than one (1) month (30 days) prior to the start date of the Activity. Cancellations between one month and two (2) weeks (14 days) prior to the start date of the Activity will be refunded Fifty-percent (50%). SYT does not refund any payments for cancellations within two (2) weeks (Fourteen (14) days) of the start date of the Activity.
Release of Liability
I agree to indemnify and hold harmless Shakespearean Youth Theatre (hereinafter referred to as SYT), its Employees, Agents, Officers, Board Members, Volunteers or any other person against loss or expense, including attorney fees, due to any bodily injury, personal injury or property damage which may result from any and all activities while participating in classes or any other activity sponsored by or conducted by SYT, or while visiting any facilities owned by, leased by, or controlled by SYT.
SYT guarantees that all equipment and facilities are in good repair and are appropriate for the purposes to which they will be put, and personnel are appropriately trained and screened.
It is understood and agreed that this Release of All Liabilities shall remain in force for not more than one year from the date of this Release. The release is recorded through a computer system, along with the date of agreement.
I am either the student named in the submission form and at least 18 years of age, or I am a parent/guardian acting on behalf of a child under 18 years of age.
Photo Release
I hereby authorize Shakespearean Youth Theatre (SYT) to publish the photographs taken of my minor child, in which he/she may be included with others, including the surrounding areas depicted in said photographs, for use in any and all media without restriction for any private or public purpose. I also grant permission to use my child’s name, if desired, in connection with any said publication.
I further grant permission to copyright, re-use and republish photographic portraits or pictures of my minor child or in which he/she may be included intact or in part, composite or altered in character or form, without restriction as to changes or transformations made, through any and all media now or hereafter known for illustration, art, promotion, advertising, trade or any other purpose whatsoever.
I acknowledge that since my child’s participation is completely voluntary, neither I nor my child nor any member of my family will receive financial compensation for the use of these photographs. I further agree that my child’s participation and the usage of these photographs confers upon me and my child no rights of ownership whatsoever.
I hereby relinquish any right that I or my minor child may have to examine or approve the completed product or products or the promotional copy or printed matter that may be used in conjunction therewith or the use to which it may be applied.
I hereby release SYT, its contractors, clients, employees, officers, legal heirs, investors, agents, representatives, and assigns from all liability for any claims by me, my minor child, my family or any third party in connection with my child’s participation, including any claims and demands ensuing from or in connection with the use of the photographs, including any and all claims for libel and invasion of privacy. I also release them from any expectation of confidentiality for the use of said photographs.
SYT uses photos of from classes, workshops, camps, and other programs primarily for print or online advertisements, archival purposes, or to share with students and families as memories. By checking ‘Agree’ and submitting this registration form, I hereby affirm that I am the parent or legal guardian of the participating minor child listed on this form. I have read the above authorization, release and agreement, prior to its execution; I fully understand the contents thereof. This agreement shall be binding upon me and my heirs, legal representatives and assigns.
